What proof will you be submitting?

A currently active/valid artist membership card (CAEA, VMA, IATSE, DGC, PGC, UBCP/ACTRA, etc.)

A program with your name credited within the past two years

A screenshot or link to a listing on any professional artist database (imdb, CBA, etc.) with a verifiable credit from within the past two years

Your name on a current staff list for an arts organization
